Course Overview:

Medication Therapy Management (MTM) is being provided by pharmacists in a variety of patient-care settings across the United States. The U.S. Surgeon General has advocated for recognition of pharmacists as providers in response to pharmacists’ expanding roles in chronic disease management. This course provides detailed information and tips of how pharmacists can provide meaningful interventions and improve patient outcomes. The most recent CMS guidelines of MTM services will be reviewed to enable pharmacists to be prepared for the new requirements and to begin incorporating those practices now. In addition, numerous case studies are presented that provide illustrative examples of pharmacist interventions and tips for how you can make a difference for the patients you serve. 

Learning Objectives:
At the conclusion of this continuing education lesson, the pharmacist will be able to:

  • Explain the most recent MTM requirements issued by the Centers for Medicare and Medicaid Services (CMS).
  • Demonstrate what is required of pharmacists to provide to patients as part of a Comprehensive Medication Review (CMR).
  • Describe innovative pharmacist interventions to improve the safety and efficacy of medication use. 
  • Provide pharmacotherapy recommendations to prescribing providers to improve chronic disease-state management.

Gina D. Moore, PharmD, MBA 

Director of Clinical Affairs, University of Colorado School of Pharmacy
Gina Moore earned her PharmD degree from the University of the Pacific Thomas J. Long School of Pharmacy and Health Sciences in Stockton, California, and her Masters in Business Administration from the University of Colorado.
She began her career in Critical Care and Nutrition Support and completed a Nutrition Support Specialty Residency at University of Colorado. In 1995, Dr. Moore joined the Healthcare division of Thomson Reuters (at that time, Micromedex) first serving in an editorial capacity, then held various positions of increasing responsibility including Director of Product Management and Director of Clinical Solutions Support. At Thomson Reuters, she led several new product development teams and consulted with a variety of healthcare organizations across the country on decision support initiatives. 

Dr. Moore joined the faculty of the University of Colorado School of Pharmacy in 2008. She coordinates or teaches the law and management courses and is involved with an intraprofessional certificate program to certify healthcare professionals in health information technology.

Joseph P. Vande Griend,

Dr. Joseph Vande Griend graduated from the University of Iowa with his Doctor of Pharmacy in 2005. He completed both PGY1 (Pharmacy Practice) and PGY2 (Family Medicine) training at the University of Colorado Hospital and School of Pharmacy. He is currently an Assistant Professor at the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences. He is also privileged to be a geriatric clinical pharmacist at the University of Colorado Seniors Clinic. At the clinic, he provides medication therapy management (MTM), performs drug therapy consultations, and serves as a preceptor for pharmacy students and residents. His research interests include Geriatric Pharmacotherapy and the provision of innovative clinical pharmacy services in primary care with a focus on the elderly population.
