| The keystone to any program of continuous quality improvement is the system of monitoring for adverse drug events. It is also the most difficult. Few pharmacies have an effective system of collecting this information and using it to reduce errors. An effective Adverse Drug Event reporting program must answer the questions of what, when, where, and how to document the needed information. In order to ensure the program is used, the staff must also understand WHY it is necessary. This program will answer each of these questions. |
| Learning Objectives: |
At the conclusion of this continuing education lesson, the pharmacist will be able to:
- Explain the purpose of reporting and documenting adverse drug events.
- Identify which adverse drug events should be documented and reported.
- Describe how adverse drug events should be documented and reported in order to make use of the information and data collected.
- Discuss how the documentation of adverse drug events leads to reduction of the risk of future medication errors.
- Identify how information regarding adverse drug events is discussed with staff members for the best use of the information.
- Recognize how reporting of adverse drug events fits into the overall system of risk management and why this is an integral part of the workflow.

He has written insurance policies and has designed quality assurance plans to reduce medication errors. Ken has taught pharmacy law and business law and has written and lectured nationally in the areas of risk management, pharmacy law and ethics. |
