The Pharmacist’s Role in the Medication Therapy Management of Chronic Pain

Presented by Kathryn Hahn, Pharm.D., DAAPM
Associate Professor, Department of Clinical Pharmacy

Kathryn Hahn earned her Pharm.D. from the University of Southern California. Before starting a pain management specialty in community pharmacy, she was an inpatient clinical pharmacy specialist in oncology and pain management. Dr. Hahn is Affiliate Faculty at Oregon State University College of Pharmacy and chair of the Oregon Pain Management Commission, which is responsible for improving pain management in Oregon through revised state policy. Currently, Dr. Hahn participates as as a community pharmacist in a unique collaborative multidisciplinary pain team that serves more than 300 chronic pain patients. She has authored numerous articles, serves as an advisor to Webscape, is on the board of the Western Pain Society and is a credentialed pain practitioner through the American Academy of Pain Management. Dr. Hahn is an active lecturer both locally and nationally.

Learning Objectives:

  1. Provide reasons to develop a pain management niche in a community pharmacy.
  2. Describe the steps in the process of developing a pain management program.
  3. Understand the concept of balance between adequate pain treatment and prevention of diversion.
  4. Describe 5 elements of medication therapy management.
  5. Complete a medication therapy review, personal medication record, medication action plan, and intervention or referral for patients with pain.
